Terms and Conditions
I
confirm that all information on this application is accurate and
complete. Permission is
granted to Decton, Inc. to verify all information in this
application and to contact current and former employers.
Permission is also granted to Decton to
complete a criminal background history.
I release such employers, Decton, Inc. from any liability and
damages resulting from the exchange of such information.
I understand that I may be required to submit
to a urinalysis or oral drug test that will be shared with Decton,
Inc., but will otherwise be kept confidential,
and the results will not be disclosed to any person at Decton, Inc.
other than those who such disclosure is necessary.
Positive test results disqualify me from employment.
Intentional falsification of any part of this application or
violation of this agreement may be grounds for termination from
Decton, Inc.
Associate Employment Agreement
1. Decton company policy is that we will make all work arrangements
with our customers. Please do not make or accept any agreements as
to your assignment, scheduling or compensation with our customers.
Our policy does not allow you to participate in any employment
arrangement with any third party contractor while employed with
Decton. A third party
contractor is a person who is working or has worked at a job site
where you are working or have worked.
This restriction on employment will be in effect for one year in the
event of your termination from Decton, Inc.
If you violate the terms of this provision, without the
written approval of Decton Inc, you agree to pay us, as liquidated
damages, an amount equal to ten percent of your most recent wage
annualized.
2. We ask that you agree to follow all work, safety and other rules
at any job site you work.
You are required to report to work with safety shoes and
customary tools and safety equipment of your trade.
3. We ask that you
notify our office immediately if:

You are going to be late for work or miss work.

You are involved in an accident at the worksite, even if the
accident seems to be minor.

Conditions exist at your job site that you believe are unsafe or may
involve exposure to hazardous or toxic substances.
-You are asked to perform work that you are not trained.
-You
are convicted of a crime.
4. Company policy does not permit the use of illegal drugs or
alcohol on the jobs site.
This includes prescription medication if it could affect job
performance. If there is
a reasonable suspicion you have violated this rule, or after an
on-site accident, we may ask you to submit to a drug and/or alcohol
test.
5. You must submit an accurately completed timecard, which is signed
by our authorized customer.
We must receive your faxed or mailed timecard by Monday at
10:00 am to have your paycheck distributed to you that week.
Timecards received after Monday at 10:00 am will be paid on a
bi-weekly pay schedule.
6. We have a No-Solicitation/No-Distribution Rule.
It is our policy that employees not solicit other employees
or our customers for any purpose at any time, and that employees not
distribute literature to other employees or customers or their
employees at anytime.
This rule does not prohibit employees from soliciting on behalf of
labor organization during times when neither employee is required to
be working, nor does it prohibit employees from distributing
literature on behalf of labor organizations during non-working
hours. We ask that you
agree and abide by this rule.
7. This employment relationship is one of “at will” employment, and
may be terminated at any time with or without cause by Decton, Inc.
I have read, understand and agree to all the above Terms and
Conditions, and the Associate Employment Agreement contained in this
application.
